PROUD PARTNERS OF:

The Rocky Mountain Care Foundation relies heavily on grants, donations and our fundraising events to fund our charitable care initiative and our “DUO” volunteer community outreach program. Remember: when you give from the heart, you are paid back tenfold.

About the Foundation

The Rocky Mountain Care Foundation (RMCF) is a non-profit, 501(c)(3), charitable organization founded in 1999. The mission of the Foundation is to:

"facilitate healthcare services

and improve the quality of life for

underserved individuals."

CHARITABLE CARE: The Rocky Mountain Care Foundation is committed to the goal of minimizing barriers that limit healthcare accessibility for individuals without financial or insurance resources. The Foundation offers a charitable care program for patients who qualify for assistance. If funds are available, the Foundation may subsidize the cost of services provided by a healthcare entity in full or at a discount, for underinsured patients with limited financial resources. Financial eligibility is determined by family size, income percentages in relation to the Federal Poverty Guidelines and extenuating circumstances. Healthcare services may include procedures, medications and/or equipment determined to be valuable and/or necessary to meet a patient's medical needs.   We have purchased eyeglasses, dentures,  hearing aids, grab bars, shower chairs, paid for hospice care, home care, co-payments, deductibles, prescriptions - whatever it takes for those in need to receive essential healthcare services.

VOLUNTEER SERVICES: Our community outreach volunteer program. "DUO" (Do Unto Others) will provide critical yet minor home repairs as well as seasonal yard maintenance for homeowners in Davis County, that are low income seniors or those living with disabilities. In formal partnerships with Easter Seals Senior Community Service Employment Program and Americorps VISTA we will be able to bring these services to our community.  It is the goal of "DUO" to help this vulnerable population remain in their homes and independent for as long as possible.  Sometimes, a neighbor's independence, depends on you.   Look for this service coming Summer of 2012!!
